Please fill out this information promptly.  To ensure the accuracy of the settlement payments, we can only send out settlement checks to clients who have completed this information.

If you will need multiple checks issued for the same Investor ID, please send an email to Christine Arnoult at [email protected] with the following information for each check:  
  • Name or account name (e.g. IRA account) that the check should be made out to
  • Amount
  • Any special information that should be on the check (ex. account #, etc.)
  • Mailing Address
